Check - In
Upon arrival, check yourself in on the lobby kiosk and update your conditions. This will let Dr. Jane know you have arrived and will maximize your treatment time. Any additional paperwork needed can be completed through the patient resources tab above under the patient forms section.
If you would like to make an appointment, please contact our office by phone or schedule online by clicking ‘Schedule/Manage Appointments’ above.
If you would like to cancel an appointment, you can do so above under the Manage Appointment link above or by leaving a message on the office phone. We ask that you provide us with at least 24 hours notice. A fee may be charged for a no show or late cancellation, and patients may be discharged from our practice for repeatedly missing scheduled appointments.
